Job Description
We are seeking a dedicated and compassionate Community Health Aide with a strong outreach focus to join our community health team. This full-time role operates Monday to Friday and is crucial in supporting patients within their homes and community settings, promoting health, and preventing illness. The successful candidate will work closely with healthcare professionals to deliver essential care, provide practical support, and connect individuals with appropriate resources, enhancing their overall well-being and contributing to a healthier Wolverhampton.
Key Responsibilities
- Conduct home visits to support patients with personal care, medication reminders, and general well-being checks.
- Assist in the implementation of care plans developed by nurses and other healthcare professionals.
- Provide practical assistance and emotional support to patients and their families.
- Educate individuals on health promotion, healthy lifestyle choices, and disease prevention.
- Facilitate access to community resources, social services, and support groups.
- Accurately record patient observations, care provided, and any changes in condition.
- Participate in community outreach programs and health awareness campaigns.
- Work collaboratively with multidisciplinary teams, including nurses, social workers, and therapists.
- Maintain professional boundaries and confidentiality at all times, adhering to all NHS policies and guidelines.

- Ability to work independently and as part of a multidisciplinary team.

- Good observational and accurate reporting skills.
- Basic IT literacy for electronic record-keeping and communication.

- Full UK driving license and access to a reliable vehicle for extensive community visits.
Preferred Qualifications
- NVQ Level 2 or 3 in Health and Social Care or an equivalent relevant qualification.
- Previous experience (at least 6 months) in a community health or social care setting.
- Knowledge of local community resources and health services available in Wolverhampton.
- First Aid certification or willingness to undertake training.
