Job Description

We are seeking a highly motivated and experienced Transplant Coordinator (RN) to join our esteemed transplant team in La Jolla, CA. This role is crucial for ensuring the seamless coordination of patient care throughout the transplant process, from evaluation to post-transplant follow-up. The successful candidate will manage a complex caseload, act as a primary resource for patients and families, and collaborate closely with multidisciplinary teams. Key Responsibilities

  • Manage and coordinate all aspects of patient care for transplant candidates and recipients, including pre-transplant evaluation, waitlist management, and post-transplant follow-up.
  • Serve as the primary point of contact and educator for patients and their families, providing comprehensive information and emotional support throughout the transplant journey.
  • Facilitate communication and collaboration among a multidisciplinary team, including physicians, surgeons, social workers, dietitians, and financial counselors.
  • Ensure compliance with all regulatory requirements and UNOS policies related to transplant patient care and documentation.
  • Participate in the on-call rotation for organ offer evaluation and donor coordination.
  • Conduct patient assessments, monitor clinical data, and identify changes in patient status requiring intervention.
  • Coordinate diagnostic testing, appointments, and procedures, ensuring timely completion.
  • Maintain accurate and thorough documentation in electronic health records and transplant databases.

Required Skills

  • Current Registered Nurse (RN) license in the State of California.
  • Minimum of 4 years of clinical nursing experience, with at least 2 years in transplant, critical care, or a relevant specialty.
  • Strong understanding of the transplant process, including pre-transplant evaluation, organ allocation, and post-transplant care.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work autonomously, prioritize tasks, and manage a complex caseload effectively.
  • Proficiency in electronic health records (EHR) systems.
  • Demonstrated ability to provide empathetic and patient-centered care.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Certified Clinical Transplant Coordinator (CCTC) certification.
  • Experience with adult solid organ transplant programs.
  • Master's degree in Nursing or a related field.
