Job Description

We are seeking a highly motivated and experienced Registered Nurse (RN) to join our esteemed team as a Transplant Coordinator. This high-responsibility role is critical to the success of our transplant program, requiring exceptional clinical acumen, organizational skills, and a profound commitment to patient care. The Transplant Coordinator will manage all aspects of the transplant process, from evaluation to post-transplant follow-up, ensuring seamless coordination and support for patients and their families. This position offers a top salary commensurate with the significant impact and expertise required.

Key Responsibilities

  • Coordinate the comprehensive evaluation process for potential transplant candidates.
  • Educate patients and families on all aspects of the transplant journey, including pre-transplant preparation, surgical procedures, and post-transplant care.
  • Manage patient listings, communicate with UNOS (United Network for Organ Sharing), and ensure compliance with all regulatory requirements.
  • Facilitate communication between patients, physicians, multidisciplinary team members, and referring providers.
  • Provide expert clinical assessment, intervention, and management for transplant patients, addressing their physical and psychosocial needs.
  • Participate in on-call rotation to manage organ offers and urgent patient needs.
  • Maintain accurate and thorough patient records and documentation.
  • Collaborate with transplant surgeons, nephrologists/hepatologists, social workers, dietitians, and pharmacists to optimize patient outcomes.

Required Skills

  • Current California Registered Nurse (RN) license.
  • Bachelor of Science in Nursing (BSN).
  • Minimum of 4 years of clinical nursing experience, with at least 2 years in transplant, critical care, nephrology, or a related specialty.
  • Strong understanding of organ transplantation principles, immunosuppression, and post-transplant complications.
  • Exceptional communication, interpersonal, and organizational skills.
  • Ability to work autonomously and collaboratively within a fast-paced, high-pressure environment.
  • Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications

  • Certified Clinical Transplant Coordinator (CCTC) certification.
  • Master's degree in Nursing or a related healthcare field.
  • Experience with adult kidney, liver, or heart transplant programs.
  • Familiarity with UNOS policies and procedures.
