Job Description
The Medication History Technician plays a critical role in patient safety and clinical care by accurately collecting and documenting patient medication histories upon admission. This position requires meticulous attention to detail, strong communication skills, and the ability to interact professionally with patients, families, and healthcare providers. The Medication History Technician will work directly with patients and their caregivers to obtain a comprehensive list of all prescription medications, over-the-counter drugs, herbal remedies, and dietary supplements, ensuring the information is accurate and readily available for clinical decision-making.
Key Responsibilities
- Interview patients and/or their families to obtain a complete and accurate medication history, including medication names, dosages, routes, frequencies, last dose taken, and indication.
- Document medication histories accurately in the electronic health record (EHR) within established timeframes.
- Verify medication histories by cross-referencing with pharmacies, previous medical records, and other available resources as needed.
- Communicate effectively with pharmacists, physicians, nurses, and other healthcare team members regarding discrepancies or unclear information in medication histories.
- Identify and escalate potential medication-related problems or safety concerns to the appropriate clinical staff.
- Maintain patient confidentiality and adhere to all HIPAA regulations.
- Participate in departmental quality improvement initiatives and training programs.
- Assist in maintaining an organized and efficient workflow within the pharmacy department.
Required Skills
- High School Diploma or GED
- Active Pharmacy Technician certification (CPhT) from the PTCB or NHA
- Minimum of 1 year of experience as a Pharmacy Technician, preferably in a hospital or clinical setting
- Excellent verbal communication and interpersonal skills
- Strong attention to detail and accuracy
- Proficiency in electronic health records (EHR) and pharmacy information systems
- Ability to work independently and as part of a team in a fast-paced environment
- Knowledge of common medication names, dosages, and routes of administration
Preferred Qualifications
- Associate's degree in a healthcare-related field
- Experience with medication reconciliation processes
- Fluency in Spanish or other languages commonly spoken in the community
- Prior experience specifically in medication history collection
