Job Description
We are seeking a highly skilled and compassionate Family Nurse Practitioner (FNP) to join a thriving private practice within the Sutter Health network in San Jose, CA. This is an exceptional opportunity for an FNP to provide comprehensive primary care services to a diverse patient population, focusing on health promotion, disease prevention, and management of acute and chronic conditions. The ideal candidate will be a proactive, patient-centered professional eager to contribute to a collaborative healthcare team and build strong, lasting relationships with patients.
Key Responsibilities
- Conduct comprehensive health assessments, including medical histories and physical examinations.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient conditions.
- Formulate and implement evidence-based treatment plans, including prescribing medications.
- Provide health education and counseling to patients and their families on disease prevention, health maintenance, and chronic disease management.
- Manage acute and chronic illnesses, performing follow-up care and making appropriate referrals.
- Maintain accurate and timely electronic health records.
- Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated patient care.
- Participate in quality improvement initiatives and continuing education to enhance clinical skills and knowledge.
Required Skills
- Current California Registered Nurse (RN) license.
- Current California Nurse Practitioner (NP) certificate.
- Current national certification as a Family Nurse Practitioner (FNP).
- Master's or Doctorate degree in Nursing (MSN/DNP) from an accredited program.
- Current DEA registration.
- Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS) certification (or ability to obtain within 6 months of hire).
- Proficiency in electronic health record (EHR) systems.
- Excellent communication, interpersonal, and problem-solving skills.
Preferred Qualifications
- Bilingual proficiency (especially Spanish).
- Experience with population health management and preventative care programs.
- Demonstrated experience in a private practice or outpatient clinic setting.
- Interest in community outreach and health initiatives.
