Job Description

Join Grand River Hospital as a Community Health Aide with an outreach focus! In this vital role, you will be instrumental in connecting community members with essential health services, promoting wellness, and providing direct support. This position is ideal for an empathetic individual passionate about making a tangible difference in people’s lives through proactive community engagement and health education. Your work will primarily involve field-based activities, visiting various community settings Monday to Friday to support health initiatives and patient navigation.

Key Responsibilities

  • Conduct outreach activities in various community settings to identify individuals in need of health services.
  • Provide information and education on available health programs, preventative care, and healthy lifestyle choices.
  • Assist individuals in navigating the healthcare system, including scheduling appointments and arranging transportation.
  • Document client interactions, assessments, and progress accurately and confidentially.
  • Collaborate with healthcare professionals, social workers, and community partners to ensure comprehensive support.
  • Support clients in completing necessary forms and applications for health-related benefits.
  • Monitor client well-being and report any significant changes or concerns to appropriate clinical staff.
  • Organize and participate in community health events and workshops.

Required Skills

  • Excellent interpersonal and communication skills, both verbal and written.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organizational skills and attention to detail.
  • Valid driver's license and access to a reliable vehicle for community travel.
  • Proficiency in basic computer applications (e.g., Microsoft Office Suite).
  • Empathy, patience, and a non-judgmental approach to client care.

Preferred Qualifications

  • Post-secondary education in a health-related field (e.g., Community Health, Social Work, Nursing Assistant).
  • Previous experience in community outreach, social services, or a healthcare setting.
  • Fluency in additional languages relevant to the local community (e.g., German, Arabic, Mandarin, Punjabi).
  • Knowledge of local community resources and health support networks.
