Job Description
We are seeking a highly motivated and experienced Registered Nurse Case Manager to join our remote team. This is a 100% remote position, allowing you to work from anywhere within the United States. As an RN Case Manager, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet an individual’s health needs. You will advocate for our members to ensure they receive appropriate, cost-effective care and services. This role requires strong clinical judgment, excellent communication skills, and a passion for helping people navigate the complexities of healthcare.
Key Responsibilities
- Conduct comprehensive assessments of members' health needs, including physical, psychosocial, and environmental factors.
- Develop, implement, and monitor individualized care plans in collaboration with members, families, and healthcare providers.
- Coordinate and facilitate access to necessary healthcare services, community resources, and support systems.
- Educate members and families on health conditions, treatment options, and self-management strategies.
- Evaluate the effectiveness of care plans and make adjustments as needed to optimize member outcomes.
- Maintain accurate and timely documentation in compliance with company policies and regulatory requirements.
- Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals.
- Identify potential barriers to care and develop strategies to overcome them.
- Ensure continuity of care and appropriate transitions between different levels of care.
- Adhere to all ethical and professional nursing standards.
Required Skills
- Active, unrestricted Registered Nurse (RN) license in at least one U.S. state (multi-state licensure preferred or ability to obtain multi-state licensure).
- Minimum of 3 years of clinical experience as an RN.
- Minimum of 2 years of case management experience.
- Proficiency in using electronic medical records (EMR) systems and other healthcare technology.
- Strong critical thinking and problem-solving abilities.
- Excellent verbal and written communication skills.
- Ability to work independently and manage a diverse caseload effectively.
- Demonstrated ability to build rapport and engage with members and their families.
- Computer proficiency, including Microsoft Office Suite.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Certified Case Manager (CCM) certification.
- Experience in managed care, home health, or utilization review.
- Bilingual skills (e.g., Spanish/English).
- Experience with telephonic case management.
