Job Description

We are seeking a dedicated and compassionate Geriatric Nurse Practitioner to join our team in Dartmouth, Nova Scotia. As a Geriatric Nurse Practitioner, you will play a critical role in providing advanced primary care to our senior residents, focusing on health promotion, disease prevention, and the management of acute and chronic conditions. Key Responsibilities

  • Conduct comprehensive health assessments, diagnose, and treat acute and chronic health conditions in geriatric residents.
  • Develop, implement, and evaluate individualized care plans in collaboration with residents, families, and the healthcare team.
  • Prescribe medications and other therapies in accordance with professional scope of practice and provincial regulations.
  • Provide health education and counseling to residents and their families on health maintenance, disease management, and preventative care.
  • Participate in interdisciplinary team meetings to coordinate resident care and enhance service delivery.
  • Act as a clinical leader and mentor to nursing staff, fostering a culture of continuous learning and excellence.
  • Maintain accurate and thorough electronic health records.
  • Contribute to quality improvement initiatives and adherence to regulatory standards.

Required Skills

  • Current registration as a Nurse Practitioner (NP) in Nova Scotia or eligibility for registration.
  • Master's or Doctorate degree in Nursing (MSN/DNP) with a specialization in Gerontology or Adult/Family Nurse Practitioner.
  • Strong clinical assessment, diagnostic, and treatment skills specific to the geriatric population.
  • Excellent communication, interpersonal, and team collaboration skills.
  • Demonstrated ability to work autonomously and make sound clinical judgments.
  • Proficiency in electronic health record (EHR) systems.

Preferred Qualifications

  • Minimum of 2 years of experience as a Nurse Practitioner, preferably in geriatric care, long-term care, or primary care settings.
  • Experience with health equity and culturally competent care.
  • Certifications in advanced cardiac life support (ACLS) and basic life support (BLS).
