Job Description
We are seeking a highly motivated and compassionate Public Health Nurse to join our dynamic team. This full-time position operates Monday to Friday, providing essential public health services within various community settings. The Public Health Nurse will be instrumental in promoting health, preventing disease, and providing health education and advocacy for individuals, families, and communities in Santa Clara County. This role involves working autonomously and collaboratively to assess community health needs, plan and implement interventions, and evaluate program effectiveness, all while adhering to public health principles and ethical standards.
Key Responsibilities
- Conduct comprehensive health assessments of individuals, families, and communities.
- Develop, implement, and evaluate health promotion and disease prevention programs.
- Provide health education and counseling on a wide range of public health topics.
- Investigate communicable disease outbreaks and implement control measures.
- Refer clients to appropriate community resources and follow up on service utilization.
- Collaborate with other healthcare professionals, community organizations, and government agencies.
- Maintain accurate and confidential client records in accordance with established guidelines.
- Participate in emergency preparedness and response activities as needed.
- Advocate for policies and programs that support community health and wellness.
Required Skills
- Valid California Registered Nurse (RN) license.
- Minimum of 3 years of experience in public health, community health, or a related clinical setting.
- Strong understanding of public health principles, practices, and epidemiology.
- Excellent communication, interpersonal, and cultural competency skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records and standard office software.
- Valid California Driver's License and access to reliable transportation.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN).
- Certification in Public Health Nursing (PHN).
- Experience working with diverse populations and vulnerable communities.
- Bilingual proficiency in Spanish, Vietnamese, Mandarin, or other languages prevalent in Santa Clara County.
