Job Description

We are seeking a diligent and meticulous Clinical Coder to join our Health Records team. In this pivotal role, you will be responsible for translating complex clinical information from patient records into standardised alpha-numeric codes using national and international classification systems (ICD-10 and OPCS-4). Your work will directly impact the Trust’s income generation, clinical governance, and strategic planning by ensuring accurate and timely data submission. This position requires exceptional attention to detail, a strong understanding of medical terminology, and the ability to work effectively within a fast-paced healthcare environment.

Key Responsibilities

  • Abstracting relevant clinical information from patient case notes and electronic systems.
  • Assigning appropriate ICD-10 and OPCS-4 codes to accurately reflect diagnoses and procedures.
  • Ensuring coding practices adhere to national standards, local policies, and governance frameworks.
  • Liaising with clinicians and other healthcare professionals to clarify complex clinical information.
  • Participating in regular audit processes to maintain coding accuracy and data quality.
  • Keeping up-to-date with changes in national coding standards and medical terminology.
  • Contributing to the continuous improvement of coding processes and systems.
  • Maintaining confidentiality and data security at all times.

Required Skills

  • Proven experience as a Clinical Coder using ICD-10 and OPCS-4.
  • Excellent understanding of medical terminology, anatomy, and physiology.
  • High level of accuracy and attention to detail.
  • Strong analytical and problem-solving skills.
  • Proficiency in using IT systems, including electronic patient records and coding software.
  • Ability to work independently and as part of a team.
  • Good communication and interpersonal skills.

Preferred Qualifications

  • Accreditation by the Institute of Health Records and Information Management (IHRIM) or equivalent.
  • Further education in health information management or related fields.
  • Experience with NHS patient administration systems (PAS).
