Job Description
We are seeking a diligent and meticulous Clinical Coder to join our Health Records team. In this pivotal role, you will be responsible for translating complex clinical information from patient records into standardised alpha-numeric codes using national and international classification systems (ICD-10 and OPCS-4). Your work will directly impact the Trust’s income generation, clinical governance, and strategic planning by ensuring accurate and timely data submission. This position requires exceptional attention to detail, a strong understanding of medical terminology, and the ability to work effectively within a fast-paced healthcare environment.
Key Responsibilities
- Abstracting relevant clinical information from patient case notes and electronic systems.
- Assigning appropriate ICD-10 and OPCS-4 codes to accurately reflect diagnoses and procedures.
- Ensuring coding practices adhere to national standards, local policies, and governance frameworks.
- Liaising with clinicians and other healthcare professionals to clarify complex clinical information.
- Participating in regular audit processes to maintain coding accuracy and data quality.
- Keeping up-to-date with changes in national coding standards and medical terminology.
- Contributing to the continuous improvement of coding processes and systems.
- Maintaining confidentiality and data security at all times.
Required Skills
- Proven experience as a Clinical Coder using ICD-10 and OPCS-4.
- Excellent understanding of medical terminology, anatomy, and physiology.
- High level of accuracy and attention to detail.
- Strong analytical and problem-solving skills.
- Proficiency in using IT systems, including electronic patient records and coding software.
- Ability to work independently and as part of a team.
- Good communication and interpersonal skills.
Preferred Qualifications
- Accreditation by the Institute of Health Records and Information Management (IHRIM) or equivalent.
- Further education in health information management or related fields.
- Experience with NHS patient administration systems (PAS).
