Job Description

We are seeking a dedicated and empathetic Community Health Aide with an outreach focus to join our dynamic team. This full-time role operates Monday to Friday, playing a crucial part in supporting community health initiatives and improving patient outcomes outside of a traditional hospital setting. The successful candidate will be a proactive individual passionate about community engagement, health education, and providing essential support to vulnerable populations across Bath and North East Somerset. This position is ideal for someone looking to make a direct impact on public health by working closely with individuals in their homes and community spaces, promoting well-being and facilitating access to healthcare services.

Key Responsibilities

  • Conduct home visits and community outreach activities to engage with patients and residents.
  • Assist in the delivery of health education programs and workshops on various topics (e.g., nutrition, hygiene, disease prevention).
  • Provide basic health screenings and vital sign measurements under supervision.
  • Support patients in navigating healthcare systems and accessing local resources.
  • Facilitate communication between patients, their families, and healthcare professionals.
  • Maintain accurate records of patient interactions and community activities.
  • Distribute health-related materials and information to community members.
  • Report any changes in patient condition or concerns to supervising staff.
  • Collaborate with other healthcare team members and community organizations.
  • Adhere to all health and safety guidelines and patient confidentiality protocols.

Required Skills

  • Excellent interpersonal and communication skills.
  • Strong empathy and a patient-centered approach.
  • Ability to work independently and as part of a team.
  • Basic understanding of health promotion principles.
  • Good organizational and record-keeping abilities.
  • Proficiency in basic computer applications (e.g., Microsoft Office).
  • Valid UK driving license and access to a vehicle (for outreach).
  • Right to work in the UK.

Preferred Qualifications

  • Previous experience in a community health, social care, or outreach role.
  • NVQ Level 2 or 3 in Health and Social Care, or equivalent qualification.
  • Knowledge of local health resources and community services in Bath and North East Somerset.
  • First Aid certification.
