Job Description

As a Patient Navigator, you will be the primary point of contact for patients, helping them access services, understand their care plans, and connect with necessary resources. Key Responsibilities

  • Serve as a primary point of contact for patients and their families, providing support and information.
  • Assist patients with scheduling appointments, referrals, and diagnostic tests.
  • Educate patients on available services, treatment plans, and necessary paperwork.
  • Connect patients with internal and external resources, including financial assistance, transportation, and support groups.
  • Maintain accurate and confidential patient records in compliance with HIPAA regulations.
  • Collaborate with clinical staff, physicians, and administrative teams to ensure seamless patient care coordination.
  • Address patient inquiries and concerns with empathy and efficiency, escalating issues when necessary.
  • Follow up with patients to ensure they understand their next steps and have access to ongoing support.

Required Skills

  • Exceptional verbal and written communication skills.
  • Strong interpersonal skills and a compassionate, empathetic approach.
  • Excellent organizational and time management abilities.
  • Basic computer proficiency (e.g., Microsoft Office Suite, electronic health records a plus).
  • Ability to work effectively in a fast-paced and dynamic healthcare environment.
  • High school diploma or GED equivalent.
  • A strong desire to learn and contribute to a patient-centered care team.

Preferred Qualifications

  • Previous experience in customer service, administrative support, or a related field (non-healthcare included).
  • Familiarity with medical terminology.
  • Bilingual proficiency (especially Spanish) is a significant advantage.
  • Associate's or Bachelor's degree in a healthcare-related field.
