Job Description
Providence Health & Services is seeking a dedicated and compassionate Palliative Care & Hospice Intern to join our Social Services team in Corona, California. This internship offers a unique opportunity for aspiring social work professionals to gain hands-on experience in providing holistic support to patients and their families navigating serious illness and end-of-life care. The intern will work under the direct supervision of licensed social workers, contributing to patient assessments, care planning, and resource navigation within a collaborative interdisciplinary team. This role is designed to provide comprehensive exposure to the psychosocial, emotional, and spiritual aspects of palliative and hospice care.
Key Responsibilities
- Assist licensed social workers in conducting psychosocial assessments of patients and families.
- Participate in the development and implementation of individualized care plans.
- Provide emotional support and counseling to patients and families under supervision.
- Facilitate communication between patients, families, and the interdisciplinary care team.
- Identify and connect patients and families with appropriate community resources and support services.
- Assist with advance care planning discussions and documentation.
- Document patient interactions and observations accurately and timely in electronic health records.
- Attend and actively participate in team meetings, case conferences, and educational sessions.
- Adhere to all ethical guidelines, confidentiality standards, and organizational policies.
Required Skills
- Currently enrolled in a Bachelor's or Master's program in Social Work, Psychology, or a related field.
- Strong interpersonal and communication skills (verbal and written).
- Empathy, compassion, and sensitivity to the needs of individuals facing serious illness and end-of-life.
- Ability to work effectively as part of an interdisciplinary team.
- Basic understanding of grief, loss, and bereavement processes.
- Proficiency in basic computer applications (e.g., Microsoft Office Suite).
- Ability to maintain strict confidentiality and professional boundaries.
Preferred Qualifications
- Prior volunteer or work experience in a healthcare, hospice, or social service setting.
- Coursework or foundational knowledge in palliative care, hospice, gerontology, or medical social work.
- Bilingual proficiency (especially Spanish) is a plus.
