Job Description

We are seeking a dedicated and compassionate Hospice Nurse (RN) to join our team in Hilo, Hawaii. This vital role involves providing direct patient care, support, and education to terminally ill patients and their families in their homes. As a Hospice Nurse, you will manage a caseload of patients, assessing their needs, developing and implementing care plans, and ensuring comfort and dignity during end-of-life care. This position requires a strong commitment to patient advocacy, excellent clinical skills, and the ability to work independently while collaborating with an interdisciplinary team. Mileage reimbursement is provided for home visits.

Key Responsibilities

  • Conduct comprehensive nursing assessments for new and existing hospice patients in their homes.
  • Develop, implement, and evaluate individualized care plans in collaboration with the patient, family, and interdisciplinary team.
  • Administer medications, manage symptoms, and provide skilled nursing care as prescribed.
  • Educate patients and families on disease progression, pain management, medication administration, and comfort measures.
  • Provide emotional and spiritual support to patients and their families, respecting their beliefs and values.
  • Monitor and document patient's condition, response to treatment, and any changes in status accurately and timely.
  • Communicate effectively with physicians, social workers, spiritual counselors, and other team members.
  • Coordinate with pharmacies, durable medical equipment providers, and other community resources.
  • Provide crisis intervention and support as needed.
  • Participate in on-call rotation as required.

Required Skills

  • Current Hawaii Registered Nurse (RN) license in good standing.
  • Minimum of 2 years of recent clinical experience as a Registered Nurse.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
  • Ability to work independently and manage a varied caseload effectively.
  • Proficiency in electronic health records (EHR) systems.
  • Valid Hawaii driver's license and reliable transportation.
  • Ability to travel regularly to patient homes within the Hilo area.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) degree.
  • Certification in Hospice and Palliative Care (CHPN).
  • Previous experience in hospice, home health, or palliative care settings.
  • Experience working with diverse patient populations.
