Job Description
We are seeking a compassionate and proactive Community Health Aide with an outreach focus to join our dedicated team. In this full-time role, Monday to Friday, you will play a crucial part in connecting with community members, providing essential health information, and facilitating access to healthcare services. This position involves active engagement in various community settings, promoting health education, and supporting preventative care initiatives. The ideal candidate will have a strong desire to serve the community, excellent communication skills, and the ability to work independently and as part of a team.
Key Responsibilities
- Conduct outreach activities in designated community areas to identify individuals in need of health services and information.
- Provide basic health education on topics such as disease prevention, healthy lifestyles, and available community resources.
- Assist individuals in navigating the healthcare system, including scheduling appointments, understanding medical instructions, and accessing social support programs.
- Distribute health-related materials and resources to community members.
- Document interactions and maintain accurate records of outreach activities and client progress.
- Collaborate with healthcare professionals, social workers, and other community organizations to ensure comprehensive client support.
- Identify and report community health trends and needs to supervisors.
- Participate in team meetings and training sessions to enhance skills and knowledge.
Required Skills
- High School Diploma or equivalent
- Minimum 1 year of experience in a community outreach, social services, or healthcare support role
- Excellent interpersonal and communication skills (verbal and written)
- Ability to work independently and manage time effectively
- Proficiency in basic computer applications (e.g., Microsoft Office Suite)
- Strong empathy and cultural sensitivity
- Valid Ontario Driver's License and access to a reliable vehicle for community travel
Preferred Qualifications
- Post-secondary education in a health-related field (e.g., Community Health Worker Certificate, Social Service Worker Diploma)
- Bilingualism (e.g., English and Italian, Russian, Tagalog, or other languages prevalent in Vaughan)
- Experience with electronic health records (EHR) systems
- Knowledge of local community resources and health initiatives
