Job Description

Hoag Memorial Hospital Presbyterian is seeking a dedicated Life Path Coordinator to join our team in Garden Grove. This unique opportunity offers a 4-day work week, allowing for an enhanced work-life balance. As a Life Path Coordinator, you will play a pivotal role in supporting our community members, patients, or employees in navigating various life transitions, setting achievable goals, and accessing resources that empower their journey. You will provide one-on-one coaching, facilitate workshops, and develop personalized action plans, contributing directly to the well-being and success of those you serve within the Hoag network.
Key Responsibilities
- Conduct one-on-one coaching sessions to assess individual needs, goals, and challenges.
- Develop personalized life path plans, including goal setting, strategy development, and progress tracking.
- Provide guidance and support for career development, wellness initiatives, educational pursuits, and personal growth.
- Connect individuals with relevant internal and external resources, services, and support networks.
- Facilitate group workshops and seminars on topics such as stress management, resilience, goal setting, and professional development.
- Monitor and evaluate the effectiveness of programs and services, making adjustments as needed.
- Maintain accurate and confidential records of client interactions and progress.
- Collaborate with other departments and community partners to enhance resource offerings.

Preferred Qualifications
- Master's degree in a relevant field.
- Certification in coaching (e.g., ICF certification).
- Experience working within a healthcare or non-profit environment.
- Knowledge of community resources in Orange County, CA.
- Bilingual proficiency (especially Spanish or Vietnamese).
