Job Description

We are seeking a detail-oriented and experienced Medication Reconciliation Technician with a strong focus on clinical data to join our team in Uptown Dallas. In this crucial role, you will be responsible for accurately compiling and reconciling patient medication histories, ensuring comprehensive and precise data for clinical decision-making. Your expertise will directly contribute to patient safety and quality of care by preventing medication errors and optimizing treatment plans. This position requires meticulous attention to detail, strong analytical skills, and the ability to work collaboratively within a fast-paced healthcare environment.

Key Responsibilities

  • Obtain and document complete and accurate medication histories from patients, families, and external healthcare providers.
  • Reconcile medication lists upon admission, transfer, and discharge, identifying and resolving discrepancies.
  • Utilize electronic health records (EHR) and other clinical data systems to gather and verify medication information.
  • Collaborate with pharmacists, physicians, nurses, and other healthcare professionals to clarify medication details and address discrepancies.
  • Educate patients and caregivers on their medication regimens and the importance of accurate medication lists.
  • Maintain up-to-date knowledge of pharmaceutical terminology, drug classifications, and medication safety practices.
  • Contribute to quality improvement initiatives related to medication reconciliation processes and clinical data integrity.
  • Participate in team meetings and training sessions to enhance skills and knowledge.

Required Skills

  • Certified Pharmacy Technician (CPhT) certification or equivalent.
  • Minimum of 2 years of experience in a hospital or clinical pharmacy setting.
  • Proven experience with medication reconciliation processes.
  • Proficiency in using electronic health record (EHR) systems.
  • Strong understanding of pharmaceutical terminology and drug classifications.
  • Excellent communication and interpersonal skills.
  • Exceptional attention to detail and accuracy.
  • Ability to work independently and as part of a multidisciplinary team.

Preferred Qualifications

  • Associate's degree in a healthcare-related field.
  • Experience with Epic EHR system.
  • Prior experience specifically focused on clinical data analysis or quality improvement.
  • Knowledge of local and federal regulations related to medication management.
