Job Description
BayCare Health System is seeking a dedicated and compassionate Gerontology Nurse Practitioner – Senior Care Specialist to join our team in New Port Richey, Florida. This crucial role involves providing comprehensive, patient-centered primary and specialty care services to our geriatric patient population, focusing on promoting health, preventing disease, and managing complex chronic conditions specific to older adults. The ideal candidate will possess a deep understanding of the unique physiological, psychological, and social needs of seniors, working collaboratively with an interdisciplinary team to enhance the quality of life for our patients.
Key Responsibilities
- Conduct comprehensive health assessments for geriatric patients, including medical history, physical examinations, and functional status evaluations.
- Diagnose, treat, and manage acute and chronic health conditions common in older adults, such as diabetes, hypertension, heart disease, dementia, and osteoporosis.
- Develop and implement individualized care plans, focusing on disease management, preventative care, health promotion, and palliative care as appropriate.
- Prescribe medications and other treatments in accordance with state and federal regulations and BayCare policies.
- Order and interpret diagnostic tests, such as lab work and imaging, to monitor patient health and diagnose conditions.
- Provide education and counseling to patients and their families regarding health conditions, medication management, lifestyle modifications, and advanced care planning.
- Collaborate effectively with physicians, nurses, social workers, therapists, and other healthcare professionals to ensure coordinated and holistic patient care.
- Maintain accurate and comprehensive electronic health records (EHR) documentation.
- Participate in quality improvement initiatives and continuing education relevant to gerontology and senior care.
Required Skills
- Current active Florida Advanced Practice Registered Nurse (APRN) license.
- Board Certification as a Gerontological Nurse Practitioner (GNP) or Family Nurse Practitioner (FNP) with significant geriatric experience.
- Master's or Doctorate Degree in Nursing from an accredited program.
- Minimum 2 years of experience as an Advanced Practice Registered Nurse, preferably in geriatrics, primary care, or internal medicine.
- Strong clinical assessment, diagnostic, and treatment skills.
- Excellent communication, interpersonal, and empathy skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- Experience with complex care coordination and transitions of care for geriatric patients.
- Additional certifications relevant to geriatric care (e.g., Certified Dementia Practitioner).
- Bilingual proficiency (especially Spanish) is a plus.
