Job Description
Providence Health Care is seeking a highly skilled and compassionate Triage / Telehealth Nurse to join our innovative remote care team. This critical role involves providing professional nursing advice, health education, and symptom management guidance to patients via telephone and virtual platforms. You will be the first point of contact for many, assessing their needs, prioritizing urgency, and directing them to appropriate care pathways, all while working from the comfort of your home. Key Responsibilities
- Conduct comprehensive patient health assessments over the phone or video conference, evaluating symptoms and medical history.
- Utilize standardized nursing protocols and clinical judgment to triage patient calls, determining the urgency and appropriate level of care.
- Provide evidence-based health information, education, and self-care advice to patients and their families.
- Coordinate referrals to physicians, specialists, emergency services, or other healthcare providers as needed.
- Document all patient interactions, assessments, and recommendations accurately and thoroughly in electronic health records.
- Collaborate with a multidisciplinary team to ensure seamless patient care and follow-up.
- Maintain patient confidentiality and adhere to all ethical and professional nursing standards.
- Participate in ongoing professional development and training to stay current with best practices in telehealth and nursing.
Required Skills
- Current Registered Nurse (RN) license in good standing (or eligible for licensure in British Columbia).
- Minimum of 2 years of recent clinical nursing experience, preferably in an acute care, emergency, or primary care setting.
- Excellent verbal and written communication skills, with the ability to convey complex medical information clearly and empathetically.
- Strong critical thinking and problem-solving abilities, especially in a remote assessment environment.
- Proficiency in using electronic health record (EHR) systems and virtual communication platforms.
- Ability to work independently and manage time effectively in a fast-paced virtual setting.
- Demonstrated commitment to patient-centered care and quality service.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Previous experience in telehealth, telephone triage, or remote patient monitoring.
- Certification in triage nursing or advanced cardiac life support (ACLS).
- Familiarity with Canadian healthcare systems and provincial health guidelines.
- Proficiency in additional languages.
