Job Description
We are seeking a dedicated and experienced Nurse Health Coach to join our remote wellness strategy team. In this flexible, full-time role, you will empower individuals to achieve their health goals through personalized coaching, education, and support. This is a 100% remote position, allowing you to work from anywhere in the US. You will leverage your nursing background to provide evidence-based guidance on lifestyle modifications, chronic disease management, and preventive care, contributing to improved health outcomes for our members.
Key Responsibilities
- Conduct virtual health coaching sessions with members, focusing on their individual health goals and challenges.
- Develop personalized health plans based on comprehensive health assessments and member preferences.
- Educate members on various health topics including nutrition, exercise, stress management, and chronic condition self-management.
- Provide motivational support and accountability to help members sustain healthy behaviors.
- Collaborate with other healthcare professionals to ensure coordinated and holistic care.
- Maintain accurate and confidential client records in compliance with HIPAA regulations.
- Participate in ongoing training and professional development to stay current with best practices in health coaching and nursing.
Required Skills
- Active Registered Nurse (RN) license in good standing (compact license preferred)
- Minimum of 3 years of clinical nursing experience
- Strong understanding of motivational interviewing techniques and behavior change theories
- Excellent verbal and written communication skills
- Proficiency in using virtual communication platforms and electronic health records (EHR)
- Ability to work independently and manage a caseload effectively in a remote environment
- Demonstrated empathy, compassion, and active listening skills
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Certification in Health Coaching (e.g., NB-HWC, CHC)
- Experience in telehealth, case management, or disease management programs
- Familiarity with population health management strategies
- Bilingual proficiency (especially Spanish)
