Job Description
We are seeking dedicated and compassionate Home Health Aides to join our growing team in Mission San Jose, Fremont, CA. As a Home Health Aide, you will provide essential personal care and support services to patients in their homes, helping them maintain their independence and enhance their quality of life. This role offers the unique opportunity to make a direct impact on patients’ daily lives, providing assistance with daily activities, monitoring health conditions, and ensuring a safe living environment. Key Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding.
- Help with mobility and transfers, including ambulation and positioning.
- Provide light housekeeping tasks that directly relate to the patient's care environment.
- Monitor and record vital signs, observe changes in patient condition, and report to the supervising nurse.
- Assist with medication reminders as directed by the care plan.
- Prepare simple meals according to dietary restrictions.
- Engage patients in conversation and provide companionship.
- Maintain a clean, safe, and organized patient environment.
- Document care provided accurately and timely.
- Adhere to all company policies, procedures, and ethical standards.
Required Skills
- Current Home Health Aide (HHA) certification in California.
- High school diploma or GED.
- Valid California Driver's License and reliable transportation.
- Proof of auto insurance.
- Ability to pass a background check and drug screening.
- Excellent communication and interpersonal skills.
- Compassionate and patient-focused approach.
- Ability to lift and move patients as required.
- Strong observational and reporting skills.
Preferred Qualifications
- Previous experience (1+ year) as a Home Health Aide or Certified Nursing Assistant (CNA).
- Familiarity with electronic health records (EHR) systems.
- Bilingual skills (e.g., Spanish, Mandarin, Cantonese) are a plus.
