Job Description
We are seeking a dedicated and enthusiastic Community Health Aide with an outreach focus to join our dynamic team in Camden. This full-time role operates Monday to Friday, providing essential support to our community health services. The successful candidate will play a vital role in connecting with individuals in their homes and local communities, assisting with health promotion, early intervention, and signposting to relevant services. You will work closely with nurses, therapists, and other healthcare professionals to ensure integrated and holistic care. This position is perfect for someone passionate about community engagement and improving health outcomes.
Key Responsibilities
- Conduct outreach visits to individuals and families in their homes or community settings.
- Assist with health assessments, data collection, and basic health screenings under supervision.
- Provide information and education on health promotion, disease prevention, and healthy lifestyle choices.
- Support individuals in accessing healthcare services, appointments, and community resources.
- Help facilitate community health events, workshops, and support groups.
- Maintain accurate and confidential client records in line with Trust policies.
- Collaborate effectively with multidisciplinary teams, including nurses, therapists, and social workers.
- Identify and report any safeguarding concerns or risks to senior staff.
- Assist with administrative tasks related to community health services.
- Adhere to all relevant NHS policies, procedures, and professional guidelines.

- Full UK driving license and access to a car for community visits.
Preferred Qualifications
- Previous experience in a healthcare or community outreach setting.
- NVQ Level 2 or 3 in Health and Social Care, or equivalent qualification.
- Knowledge of local community resources and services in Camden.
- Experience working with vulnerable populations.
- First Aid qualification.
