Job Description

We are seeking a diligent and detail-oriented Medication Reconciliation Technician with a strong focus on clinical data to join our pharmacy team. In this critical role, you will be responsible for obtaining and documenting an accurate medication history for patients upon admission, transfer, and discharge. Your work will directly support patient safety by preventing medication errors and ensuring seamless care transitions. This position requires exceptional organizational skills, a deep understanding of pharmaceutical terminology, and the ability to meticulously manage and interpret clinical data from various sources.

Key Responsibilities

  • Interview patients and/or their family members to obtain a complete and accurate list of all medications, including prescriptions, over-the-counter drugs, herbal remedies, and dietary supplements.
  • Verify medication histories through physician offices, pharmacies, and electronic health records (EHRs).
  • Document medication histories accurately and promptly in the patient's electronic medical record, adhering to established protocols and standards.
  • Identify discrepancies and potential medication issues, escalating concerns to pharmacists or prescribing providers as necessary.
  • Collaborate effectively with pharmacists, nurses, physicians, and other healthcare professionals to ensure comprehensive and precise medication information.
  • Educate patients on the importance of maintaining an accurate medication list and provide basic medication-related information as appropriate.
  • Contribute to quality improvement initiatives related to medication reconciliation processes and clinical data integrity.

Required Skills

  • Certified Pharmacy Technician (CPhT) certification.
  • Minimum of 2 years of experience in a hospital or clinical pharmacy setting, with specific experience in medication reconciliation.
  • Proficiency in electronic health record (EHR) systems, preferably Epic.
  • Strong knowledge of pharmaceutical calculations, drug names (generic and brand), and dosage forms.
  • Exceptional attention to detail and accuracy.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Demonstrated ability to manage and interpret clinical data effectively.

Preferred Qualifications

  • Associate's or Bachelor's degree in a healthcare-related field.
  • Experience with data analysis or quality improvement projects in a healthcare setting.
  • Familiarity with regulatory requirements for medication management (e.g., Joint Commission, CMS).
