Job Description
We are seeking an enthusiastic and experienced Nurse Health Coach to join our innovative remote wellness team. This vital role focuses on empowering our members to achieve their health and wellness goals through personalized coaching, education, and support. As a Remote Nurse Health Coach, you will leverage your clinical expertise to guide individuals through lifestyle changes, chronic condition management, and preventive health strategies, all from the comfort of a home-based office. This position is 100% remote, offering flexibility while maintaining high standards of care and engagement. You will be instrumental in promoting a culture of health and well-being within our community of members.
Key Responsibilities
- Provide one-on-one and group health coaching remotely, utilizing telephonic and virtual platforms.
- Develop personalized wellness plans based on individual health assessments, goals, and evidence-based practices.
- Educate members on various health topics including nutrition, exercise, stress management, sleep hygiene, and chronic disease prevention.
- Support members in setting realistic goals, overcoming barriers, and building sustainable healthy habits.
- Monitor member progress, adjust coaching strategies as needed, and document all interactions accurately in electronic health records.
- Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive member care.
- Participate in ongoing training and professional development to stay current with health coaching best practices and clinical guidelines.
- Maintain strict confidentiality and adhere to all HIPAA regulations and company policies.
Required Skills
- Current Registered Nurse (RN) license in the state of California.
- Minimum of 3 years of clinical nursing experience.
- Strong understanding of health behavior change theories and motivational interviewing techniques.
- Excellent verbal and written communication skills, with the ability to convey complex health information clearly and empathetically.
- Proficiency in using electronic health records (EHR) systems and virtual communication platforms.
- Ability to work independently and manage a caseload effectively in a remote environment.
- Demonstrated ability to build rapport and trust with diverse populations.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) or higher.
- Certification in Health Coaching (e.g., NBCHWC, ACE, Wellcoaches).
- Experience in telehealth or remote patient care.
- Background in chronic disease management, public health, or health education.
- Bilingual proficiency (especially Spanish) is a plus.
