Job Description

We are seeking an experienced and highly dedicated Registered Nurse (RN) to join our esteemed transplant team as a Transplant Coordinator. This high-responsibility role is critical to the success of our transplant program, involving comprehensive coordination of patient care across the transplant continuum – from evaluation to post-transplant follow-up. The successful candidate will serve as a primary liaison for patients, families, and multidisciplinary healthcare teams, ensuring seamless communication and exceptional patient outcomes. This position offers a top salary commensurate with the significant expertise and commitment required. This is an on-site position based in Cedar Rapids, Iowa.

Key Responsibilities

  • Manage and coordinate all aspects of patient care for transplant candidates and recipients.
  • Conduct comprehensive pre-transplant evaluations and patient education.
  • Facilitate communication between patients, families, referring physicians, and the transplant team.
  • Coordinate diagnostic tests, consultations, and patient listings.
  • Provide ongoing patient education regarding transplant procedures, medications, and lifestyle adjustments.
  • Monitor patient progress and identify potential complications, reporting to physicians as needed.
  • Participate in multidisciplinary team meetings and transplant selection committees.
  • Ensure compliance with all regulatory requirements and UNOS guidelines.
  • Document all patient encounters and interventions accurately in the electronic health record.
  • Serve as a resource and advocate for transplant patients and their families.

Required Skills

  • Current Registered Nurse (RN) license in the state of Iowa.
  • Bachelor of Science in Nursing (BSN) required.
  • Minimum of 4 years of clinical nursing experience, preferably in critical care, nephrology, or transplant.
  • Demonstrated knowledge of transplant principles and practices.
  • Exceptional organizational and time management skills.
  • Superior communication and interpersonal skills, with the ability to empathize and educate.
  • Proficiency in electronic health record systems.
  • Ability to work independently and collaboratively in a fast-paced environment.

Preferred Qualifications

  • Certified Clinical Transplant Coordinator (CCTC) certification.
  • Master's degree in Nursing (MSN) or related field.
  • Previous experience as a Transplant Coordinator.
  • Fluency in additional languages.
