Job Description
We are seeking a dedicated and compassionate Public Health Nurse to join our team in Vacaville, CA. This full-time position operates Monday through Friday, focusing on community health initiatives, disease prevention, and health promotion. The Public Health Nurse will provide essential services, including health assessments, education, case management, and outreach to individuals and families across various community settings. This role involves working within a collaborative environment, adhering to public health regulations and best practices, and actively contributing to the overall health improvement of Solano County residents.
Key Responsibilities
- Conduct health assessments and screenings for individuals and families in various community settings.
- Develop, implement, and evaluate health education programs and materials on topics such as immunizations, communicable diseases, chronic disease prevention, and maternal-child health.
- Provide case management for clients with complex health needs, coordinating services with other health and social agencies.
- Investigate and control communicable diseases through surveillance, contact tracing, and outbreak management.
- Participate in emergency preparedness and response activities, including disease surveillance and mass prophylaxis clinics.
- Maintain accurate and confidential client records in compliance with HIPAA and other regulatory standards.
- Collaborate with community partners, schools, and healthcare providers to identify health needs and implement targeted interventions.
- Advocate for public health policies and programs that promote health equity and access to care.
Required Skills
- Current California Registered Nurse (RN) license
- Valid California Driver's License and reliable transportation
- Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN)
- Strong clinical assessment and critical thinking skills
- Excellent communication, interpersonal, and cultural competency skills
- Ability to work independently and as part of a multidisciplinary team
- Proficiency in electronic health records (EHR) systems
Preferred Qualifications
- Public Health Nurse (PHN) Certificate issued by the California Board of Registered Nursing
- Experience working in a community health or public health setting
- Bilingual proficiency (especially Spanish, Tagalog, or Vietnamese)
- Experience with health program planning and evaluation
- Knowledge of local community resources and health disparities
