Job Description
We are seeking an empathetic and experienced Nurse Health Coach to join our remote wellness strategy team. In this flexible role, you will leverage your clinical expertise to guide and motivate members toward achieving their health goals. This is a 100% remote position, allowing you to make a significant impact from anywhere, connecting with diverse individuals to develop personalized wellness plans, provide education, and foster lasting behavior change. Key Responsibilities
- Conduct comprehensive health assessments and identify member health risks and opportunities.
- Develop personalized health and wellness plans based on member needs, goals, and clinical guidelines.
- Provide one-on-one remote coaching and education on various health topics, including chronic disease management, nutrition, exercise, stress management, and medication adherence.
- Utilize motivational interviewing techniques to empower members to set realistic goals and overcome barriers to success.
- Monitor and track member progress, adjusting coaching strategies as needed to ensure optimal outcomes.
- Collaborate with a multidisciplinary team to provide integrated care and support for members.
- Maintain accurate and confidential documentation of all member interactions and progress.
- Stay current with evidence-based health coaching practices and clinical guidelines.
Required Skills
- Active Registered Nurse (RN) license in good standing (compact license preferred)
- Minimum 3 years of clinical nursing experience
- Strong communication, interpersonal, and active listening skills
- Proficiency in motivational interviewing and behavior change strategies
- Excellent organizational and time management abilities
- Comfortable with technology and virtual communication platforms
- Ability to work independently and as part of a remote team
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Certification in Health Coaching (e.g., NBCHWC, Wellness Coach)
- Experience in a remote or telehealth environment
- Experience in chronic disease management, population health, or preventative care
