Job Description
Kaiser Permanente is seeking a dedicated and compassionate Nurse Health Coach to join our innovative Remote Wellness Strategy team. This is a unique opportunity for a licensed Registered Nurse to leverage their clinical expertise in a flexible, remote environment, empowering individuals to achieve their health and wellness goals. As a Health Coach, you will provide personalized guidance, education, and support to members, focusing on preventive care, chronic disease management, and overall well-being. This role is crucial in promoting healthy lifestyles and improving health outcomes through virtual interactions.
Key Responsibilities
- Conduct virtual health coaching sessions with members to assess health status, identify risk factors, and set personalized health goals.
- Develop and implement individualized wellness plans based on evidence-based practices and member-specific needs.
- Educate members on various health topics, including nutrition, exercise, stress management, medication adherence, and chronic disease self-management.
- Provide empathetic support and motivational interviewing techniques to empower members to make sustainable lifestyle changes.
- Collaborate with interdisciplinary healthcare teams to coordinate care and ensure a holistic approach to member wellness.
- Maintain accurate and confidential documentation of all coaching interactions and progress in electronic health records.
- Participate in ongoing training and professional development to stay current with health coaching best practices and clinical guidelines.
- Utilize technology effectively for virtual communication, scheduling, and health data tracking.
Required Skills
- Current and unrestricted Registered Nurse (RN) license in California.
- Minimum of 3 years of clinical nursing experience.
- Excellent verbal and written communication skills.
- Strong empathetic listening and motivational interviewing skills.
- Proficiency in using electronic health records (EHR) systems and virtual communication platforms.
- Ability to work independently and manage a caseload in a remote setting.
- Demonstrated knowledge of health promotion, disease prevention, and chronic condition management principles.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Certification in Health Coaching (e.g., NB-HWC, CHWC).
- Experience in telehealth or remote patient engagement.
- Familiarity with Kaiser Permanente's integrated healthcare model.
- Bilingual proficiency (e.g., Spanish) is a plus.
