Job Description
Join Kaiser Permanente as a dedicated Hospice Nurse (RN) specializing in home visits. In this vital role, you will provide compassionate, skilled nursing care to patients with life-limiting illnesses and their families in their homes throughout the Vallejo area. You will be responsible for assessing patient needs, developing and implementing individualized care plans, managing pain and symptoms, and providing emotional and spiritual support. Key Responsibilities
- Conduct initial and ongoing comprehensive assessments of patient and family needs.
- Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
- Provide direct nursing care, including medication administration, wound care, and symptom management.
- Educate patients and families on disease progression, comfort measures, and end-of-life care.
- Provide emotional, spiritual, and psychosocial support to patients and their families.
- Maintain accurate and timely documentation of patient visits and care provided.
- Communicate effectively with physicians, family members, and other healthcare team members.
- Participate in on-call rotation as required.
- Adhere to all ethical and professional nursing standards and regulatory requirements.
- Advocate for patient comfort and quality of life.
Required Skills
- Current unrestricted Registered Nurse (RN) license in California.
- Minimum of 2 years of clinical nursing experience, with at least 1 year in hospice, home health, or palliative care.
- Current Basic Life Support (BLS) certification.
- Demonstrated ability to assess, plan, implement, and evaluate patient care.
- Excellent communication and interpersonal skills.
- Strong organizational and time management abilities.
- Valid driver's license and reliable transportation.
- Proficiency in electronic health records (EHR) systems.
- Ability to work independently and as part of a multidisciplinary team.
- Empathy, compassion, and sensitivity to the needs of terminally ill patients and their families.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Certified Hospice and Palliative Nurse (CHPN) certification.
- Experience with Kaiser Permanente's specific EHR system.
- Familiarity with the Vallejo community and local resources.
