Job Description
We are seeking a dedicated and compassionate Gerontology Nurse Practitioner – Senior Care Specialist to join our team in Pomona, California. In this vital role, you will provide comprehensive primary and specialized healthcare to our senior patient population, focusing on health promotion, disease prevention, and the management of chronic and acute conditions common in older adults. You will collaborate with an interdisciplinary team to develop individualized care plans, advocate for patient needs, and ensure the highest quality of life for our seniors. Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical examinations, health history, and functional status evaluations.
- Diagnose and treat acute and chronic illnesses in older adults, emphasizing age-appropriate care strategies.
- Order and interpret diagnostic tests, prescribe medications, and manage patient treatment plans in collaboration with supervising physicians.
- Provide health education, counseling, and support to patients and their families on topics such as disease management, nutrition, medication adherence, and end-of-life care.
- Develop and implement individualized care plans, regularly evaluating and adjusting them based on patient needs and outcomes.
- Collaborate with physicians, social workers, physical therapists, and other healthcare professionals to ensure integrated and holistic patient care.
- Participate in care coordination activities, including transitions of care and referrals to specialists or community resources.
- Maintain accurate and thorough electronic health records.
- Stay current with best practices and advancements in geriatric nursing and healthcare.
- Adhere to all legal, ethical, and professional standards of nursing practice.
Required Skills
- Current California Registered Nurse (RN) license.
- Current California Nurse Practitioner (NP) license.
- AANP or ANCC certification as an Adult-Gerontology Nurse Practitioner (AGNP) or Gerontological Nurse Practitioner (GNP).
- Master's or Doctoral degree from an accredited Nurse Practitioner program.
- Valid DEA registration.
- Current Basic Life Support (BLS) certification.
- Minimum of 2 years of experience as a Nurse Practitioner, with significant focus on geriatric or adult patient care.
- Demonstrated clinical expertise in assessing, diagnosing, and treating complex conditions in older adults.
- Excellent communication, interpersonal, and collaboration skills.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- Bilingual proficiency (English/Spanish).
- Experience in a large integrated healthcare system.
- Additional certifications in specialized areas of geriatric care (e.g., palliative care, wound care).
