Job Description
We are seeking a dedicated and compassionate Community Health Aide with a strong outreach focus to join our team. This full-time position operates Monday to Friday and is crucial in connecting community members with vital health resources and services. The successful candidate will play a pivotal role in supporting our clients’ well-being through direct engagement, education, and facilitation of access to care within various community settings. Key Responsibilities
- Conduct outreach activities in diverse community settings to identify individuals in need of health services.
- Provide basic health education and information on available programs and resources to community members.
- Assist clients with navigating the healthcare system and accessing appointments, transportation, and other support services.
- Facilitate client intake processes and assist with maintaining accurate client records.
- Collaborate with an interdisciplinary team, including nurses, social workers, and other health professionals, to ensure holistic client care.
- Support the planning and execution of health promotion events and workshops.
- Adhere to all organizational policies, procedures, and ethical guidelines regarding client confidentiality and service delivery.
- Report observations and client needs to supervisory staff and team members.
Required Skills
- High School Diploma or equivalent; post-secondary education in a health-related field is an asset.
- Previous experience in a community outreach, healthcare support, or social service role.
- Excellent interpersonal and communication skills, with the ability to build rapport with diverse populations.
- Demonstrated empathy, patience, and a non-judgmental approach to client interaction.
- Ability to work independently and as part of a collaborative team.
- Valid Ontario Class G Driver’s License and access to a reliable vehicle for outreach activities.
- Proficiency in basic computer applications (e.g., Microsoft Office Suite).
Preferred Qualifications
- Certification as a Community Health Worker or Health Care Aide.
- Bilingualism (English and French, or English and another language prominent in the Windsor-Essex community).
- Familiarity with local community resources and health support networks in Windsor-Essex.
- Experience working with vulnerable populations or individuals facing barriers to healthcare access.
