Job Description

Key Responsibilities
- Provide personal care, including bathing, dressing, grooming, and mobility assistance.
- Assist patients with light housekeeping tasks, meal preparation, and feeding.
- Remind patients to take medications as prescribed and assist with self-administration.
- Monitor and record vital signs, reporting any changes to the nursing staff.
- Engage patients in conversation and provide companionship.
- Ensure a clean, safe, and organized home environment for patients.
- Document care provided accurately and in a timely manner.
- Adhere to all Kaiser Permanente policies, procedures, and safety protocols.
Required Skills
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification valid in California.
- Minimum 6 months of experience in a home health or direct patient care setting.
- Demonstrated ability to provide compassionate and patient-centered care.
- Excellent verbal and written communication skills.
- Ability to work independently and manage time effectively.
- Valid driver's license and reliable transportation.
- Current Basic Life Support (BLS) certification.
Preferred Qualifications
- Experience with electronic health records (EHR) systems.
- Bilingual proficiency (e.g., Spanish, Tagalog) to serve diverse patient populations.
- Previous experience working with patients with dementia, Alzheimer's, or hospice care needs.
- Proven ability to adapt to varying patient needs and home environments.
