Job Description

We are seeking a compassionate and dedicated Entry-Level Patient Navigator to join our healthcare team in Murrieta. In this vital support role, you will be the bridge between patients and their healthcare journey, guiding them through the complexities of the medical system. This position is ideal for individuals with a strong desire to help others, excellent communication skills, and a foundational understanding of patient support within a healthcare setting. You will play a crucial part in ensuring patients receive timely and appropriate care, understand their treatment plans, and feel supported throughout their experience with Kaiser Permanente.

Key Responsibilities

  • Assist patients in navigating the healthcare system, including scheduling appointments, understanding referrals, and accessing necessary services.
  • Provide non-clinical support and information to patients regarding their care plans, clinic procedures, and available resources.
  • Act as a point of contact for patients, addressing their questions or concerns and escalating complex issues to appropriate clinical staff.
  • Maintain accurate and confidential patient records, ensuring all interactions are documented thoroughly and in compliance with HIPAA.
  • Collaborate with physicians, nurses, and administrative staff to ensure seamless patient care coordination.
  • Educate patients on preventive care, health maintenance, and community health programs.
  • Identify and address potential barriers to care, working proactively to find solutions for patients.
  • Demonstrate empathy and cultural sensitivity while interacting with a diverse patient population.

Required Skills

  • High School Diploma or GED equivalent.
  • Exceptional interpersonal and communication skills (written and verbal).
  • Strong organizational skills and attention to detail.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Proficiency in basic computer applications and electronic health records (EHR) systems.
  • Demonstrated ability to maintain patient confidentiality (HIPAA compliance).
  • Empathy, patience, and a genuine desire to help others.

Preferred Qualifications

  • Associate's or Bachelor's degree in a healthcare-related field (e.g., Public Health, Healthcare Administration).
  • Previous experience in a customer service or patient-facing role, preferably in healthcare.
  • Bilingual proficiency (especially Spanish) to support our diverse patient community.
