Job Description

Join our dedicated team as a Hospice Nurse (RN) and provide essential, compassionate care to patients and their families in their homes across Chittenden and Grand Isle Counties. This vital role involves managing a caseload of hospice patients, conducting comprehensive assessments, developing and implementing individualized care plans, and providing expert symptom management and emotional support. You will work autonomously while having the full backing of an interdisciplinary team, ensuring that patients receive the highest quality of end-of-life care. This position includes mileage reimbursement for travel to patient homes, acknowledging the critical nature of your home visits.

Key Responsibilities

  • Conduct initial and ongoing comprehensive nursing assessments of patients in their homes.
  • Develop, implement, and evaluate individualized hospice care plans in collaboration with the interdisciplinary team, patient, and family.
  • Provide skilled nursing care, including medication administration, wound care, and symptom management.
  • Educate patients and families on disease progression, comfort measures, and self-care techniques.
  • Provide emotional and spiritual support to patients and families, addressing psychosocial needs.
  • Coordinate care with physicians, social workers, spiritual counselors, and other team members.
  • Maintain accurate and timely electronic medical records in compliance with regulatory standards.
  • Participate in on-call rotation as required.
  • Adhere to all VNA policies, procedures, and ethical guidelines.

Required Skills

  • Valid Registered Nurse (RN) license in Vermont.
  • Minimum of 2 years of clinical nursing experience, preferably in hospice, palliative care, home health, or acute care.
  • Current CPR certification.
  • Demonstrated strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach.
  • Ability to work independently and manage a caseload effectively.
  • Proficiency in electronic health record (EHR) systems.
  • Valid driver's license and reliable transportation.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Certified Hospice and Palliative Nurse (CHPN) certification.
  • Experience with grief and bereavement support.
  • Familiarity with Vermont's healthcare system and community resources.
