Job Description

We are actively seeking a dedicated, compassionate, and skilled LPN to provide exceptional direct patient care to our diverse resident population. Key Responsibilities

  • Administer prescribed medications and treatments in accordance with established policies and procedures.
  • Monitor, record, and report changes in residents' vital signs, symptoms, and behaviors to the nursing supervisor or physician.
  • Accurately document all patient care services, observations, and interventions in electronic health records (EHR).
  • Assist residents with activities of daily living (ADLs) including personal hygiene, mobility, and feeding, as needed.
  • Collaborate effectively with Registered Nurses (RNs), physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
  • Educate patients and their families on health maintenance, disease prevention, and post-discharge care instructions.
  • Ensure a clean, safe, and organized environment for residents, adhering to infection control protocols.
  • Participate actively in care planning meetings, interdisciplinary team discussions, and staff development programs.
  • Respond promptly and appropriately to medical emergencies and resident calls.

Required Skills

  • Current and unrestricted Licensed Practical Nurse (LPN) license in the state of California.
  • Valid Basic Life Support (BLS) certification from the American Heart Association or equivalent.
  • Demonstrated proficiency in medication administration, wound care, and general nursing procedures.
  • Excellent verbal and written communication skills with the ability to interact professionally with residents, families, and staff.
  • Strong critical thinking and problem-solving abilities in a clinical setting.
  • Ability to work effectively both independently and as part of a collaborative healthcare team.
  • Compassionate, patient-centered approach to care with a strong sense of empathy.
  • Ability to adapt to a fast-paced and evolving healthcare environment.

Preferred Qualifications

  • One (1) year of experience as an LPN in a skilled nursing facility, rehabilitation center, or long-term care setting.
  • Experience with electronic health records (EHR) systems, preferably PointClickCare.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Demonstrated ability to mentor and precept new team members or nursing students.
  • Fluency in additional languages (e.g., Spanish, Vietnamese) relevant to the local community.
