Job Description

In this vital hybrid role, you will be the first point of contact for members seeking care, conducting initial assessments, providing guidance, and connecting them with appropriate medical professionals. Key Responsibilities
- Conduct initial health assessments and screenings for members via online platforms (chat, video, phone).
- Gather pertinent information regarding symptoms, medical history, and current needs.
- Provide clear, concise, and empathetic communication to members, offering reassurance and guidance.
- Utilize established protocols and decision-making tools to determine appropriate next steps for care.
- Facilitate seamless handoffs to nurses, doctors, or other specialists as required.
- Educate members on available resources, self-care options, and appointment scheduling processes.
- Document all interactions and assessments accurately and efficiently in the electronic health record system.
- Maintain strict confidentiality of all member information in compliance with HIPAA regulations.
- Participate in ongoing training and professional development to enhance skills and knowledge.
- Collaborate effectively with a multidisciplinary healthcare team to ensure coordinated care.
Required Skills
- High school diploma or equivalent.
- Exceptional verbal and written communication skills.
