Job Description

Key Responsibilities

  • Conduct initial health assessments and screenings for members via online platforms (chat, video, phone).
  • Gather pertinent information regarding symptoms, medical history, and current needs.
  • Provide clear, concise, and empathetic communication to members, offering reassurance and guidance.
  • Utilize established protocols and decision-making tools to determine appropriate next steps for care.
  • Facilitate seamless handoffs to nurses, doctors, or other specialists as required.
  • Educate members on available resources, self-care options, and appointment scheduling processes.
  • Document all interactions and assessments accurately and efficiently in the electronic health record system.
  • Maintain strict confidentiality of all member information in compliance with HIPAA regulations.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
  • Collaborate effectively with a multidisciplinary healthcare team to ensure coordinated care.

Required Skills

  • High school diploma or equivalent.
  • Exceptional verbal and written communication skills.

  • Basic computer proficiency and ability to navigate online systems.
