Job Description
We are seeking a dedicated and compassionate Gerontology Nurse Practitioner – Senior Care Specialist to join our esteemed team. In this vital role, you will provide comprehensive primary and specialized healthcare services to our senior patient population, focusing on promoting health, preventing disease, and managing acute and chronic conditions. You will work collaboratively with a multidisciplinary team to ensure the highest standard of patient-centered care, emphasizing dignity, respect, and quality of life for older adults.
Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical examinations, cognitive screenings, and functional assessments.
- Diagnose and treat acute and chronic health conditions common in older adults, such as diabetes, heart disease, dementia, and arthritis.
- Develop and implement individualized care plans, including medication management, lifestyle modifications, and referrals to specialists.
- Provide health education and counseling to patients and their families on topics such as disease prevention, healthy aging, and advance care planning.
- Collaborate with physicians, registered nurses, social workers, and other healthcare professionals to ensure coordinated and holistic patient care.
- Order and interpret diagnostic tests, such as lab work and imaging studies.
- Participate in quality improvement initiatives and continuous professional development.
- Document patient encounters accurately and in a timely manner within electronic health records.
- Serve as a resource and advocate for senior patients and their caregivers.
Required Skills
- Current Advanced Practice Registered Nurse (APRN) license in Texas.
- Current certification as a Gerontology Nurse Practitioner (GNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- Master's or Doctorate degree in Nursing from an accredited program.
- Prescriptive authority in Texas.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
- 2+ years of experience as a Gerontology Nurse Practitioner.
- Experience in a hospital, long-term care, or primary care setting.
- Bilingual proficiency (English/Spanish).
