Job Description
Are you an experienced Registered Nurse passionate about proactive health management and empowering individuals? Humana is seeking a compassionate and highly motivated Nurse Health Coach to join our remote wellness strategy team. In this flexible role, you will provide personalized health coaching and education to members, guiding them toward achieving their health goals from the comfort of their own homes. This position requires a proactive approach to identifying health risks, developing customized wellness plans, and fostering sustained behavioral change through virtual interactions. Leverage your clinical expertise to make a tangible difference in the lives of our members, supporting them in navigating complex health challenges and optimizing their overall well-being. Key Responsibilities
- Conduct comprehensive health assessments and develop individualized care plans for members.
- Provide telephonic and virtual health coaching to educate members on chronic disease management, preventive care, and healthy lifestyle choices.
- Monitor member progress, adjust health plans as needed, and provide ongoing support and motivation.
- Collaborate with interdisciplinary teams, including physicians and care coordinators, to optimize member outcomes.
- Document all member interactions and progress accurately in the electronic health record system.
- Utilize motivational interviewing techniques to encourage member engagement and self-management.
- Stay current with evidence-based health coaching practices and clinical guidelines.
Required Skills
- Active, unencumbered Registered Nurse (RN) license in your state of residence.
- Minimum 2 years of clinical nursing experience.
- Strong communication, interpersonal, and motivational interviewing skills.
- Proficiency in using electronic health records (EHR) and virtual communication platforms.
- Ability to work independently and manage a caseload effectively in a remote environment.
- Demonstrated knowledge of chronic disease management and preventive health principles.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) degree.
- Certified Health Coach (CHC) certification from a recognized organization (e.g., NBHWCC).
- Experience in telephonic health coaching or case management.
- Bilingual proficiency (English and Spanish) is a plus.
- Experience with population health management strategies.
