Job Description
We are seeking a dedicated and detail-oriented Medication History Technician to join our hospital-based team. This role is critical in ensuring accurate and comprehensive medication histories for our patients upon admission, contributing directly to patient safety and quality of care. As a Medication History Technician, you will play a vital role in patient care by obtaining and documenting medication histories through direct patient interviews, family interviews, and pharmacy record reviews. This position offers significant clinical exposure within a dynamic hospital environment, working closely with pharmacists, nurses, and physicians.
Key Responsibilities
- Obtain accurate and complete medication histories from patients, their families, and/or caregivers upon admission or at other designated points of care.
- Utilize various resources, including electronic health records, retail pharmacy databases, and patient interviews, to gather comprehensive medication information.
- Document medication history in the electronic health record with meticulous attention to detail, including drug name, dosage, frequency, route, and last dose taken.
- Clarify discrepancies or incomplete information with patients, prescribers, and pharmacies.
- Communicate effectively with pharmacists, physicians, and nursing staff regarding medication history findings and any identified discrepancies.
- Participate in medication reconciliation processes to ensure continuity of care.
- Educate patients and families on medication-related topics as needed, under the guidance of a pharmacist.
- Maintain confidentiality of patient information and adhere to all HIPAA regulations.
- Contribute to quality improvement initiatives related to medication safety and history processes.
Required Skills
- High School Diploma or GED
- Active Pharmacy Technician certification (CPhT) from PTCB or NHA
- Current state Pharmacy Technician licensure/registration (if required by state)
- Minimum of 1 year of experience as a Pharmacy Technician, preferably in a hospital or retail setting with significant patient interaction
- Excellent verbal communication and interpersonal skills
- Strong attention to detail and accuracy
- Ability to work independently and as part of a multidisciplinary team
- Proficiency with electronic health records (EHR) and pharmacy information systems
Preferred Qualifications
- Associate's or Bachelor's degree in a healthcare-related field
- Experience specifically in medication history gathering or medication reconciliation
- Familiarity with medical terminology and common drug classes
- Bilingual skills (especially Spanish) are a plus
