Job Description
Join Empath Health as a Hospice Care Assistant and become a vital part of a team committed to providing exceptional end-of-life care. As a Hospice Care Assistant, you will provide personal care, comfort, and support to patients in their homes, in residential facilities, or in our care centers. You will work closely with nurses and other healthcare professionals to ensure the highest quality of care, focusing on the patient’s physical, emotional, and spiritual well-being. Key Responsibilities
- Provide personal care to patients, including bathing, dressing, grooming, and feeding, according to the individualized care plan.
- Assist patients with mobility, transfers, and positioning to ensure comfort and prevent skin breakdown.
- Monitor and report changes in patient condition to the supervising nurse promptly.
- Assist with light housekeeping tasks within the patient's immediate environment to maintain cleanliness and order.
- Offer emotional support and companionship to patients and their families, fostering a comforting and respectful atmosphere.
- Document care provided accurately and in a timely manner, adhering to Empath Health's standards.
- Maintain a clean, safe, and organized patient care area.
- Adhere to all hospice policies, procedures, and safety guidelines.
Required Skills
- Certified Nursing Assistant (CNA) certification current in the state of Florida.
- Minimum of 1 year of experience in a healthcare setting, preferably in hospice or home health.
- Exceptional empathy, compassion, and a patient-centered approach to care.
- Strong interpersonal and communication skills, both verbal and written.
- Ability to work independently and as part of a multidisciplinary team.
- Physical ability to perform duties, including lifting, bending, and standing for extended periods.
- Valid driver's license and reliable transportation.
- Current Basic Life Support (BLS) certification.
Preferred Qualifications
- Experience specifically in hospice or palliative care.
- Knowledge of end-of-life care principles and practices.
- Bilingual skills (e.g., Spanish, Haitian Creole) are a plus.
