Job Description
We are seeking a dedicated and compassionate Nurse Health Coach to join our remote team. This role is pivotal in guiding and supporting individuals towards achieving their health and wellness goals through personalized coaching, education, and strategic planning. You will leverage your nursing expertise to empower clients to make sustainable lifestyle changes, manage chronic conditions, and navigate the healthcare system more effectively. This is a flexible, 100% remote position, allowing you to make a significant impact from anywhere.
Key Responsibilities
- Provide one-on-one health coaching sessions via phone or video conference to a diverse client base.
- Develop personalized wellness plans based on client health assessments, goals, and evidence-based practices.
- Educate clients on various health topics, including nutrition, exercise, stress management, chronic disease prevention, and medication adherence.
- Motivate and empower clients to set realistic goals and take actionable steps towards improved health.
- Monitor client progress, provide ongoing support, and adjust coaching strategies as needed.
- Collaborate with healthcare providers and other team members to ensure coordinated care.
- Maintain accurate and confidential client records in compliance with HIPAA and company policies.
- Stay current with health coaching best practices, medical advancements, and wellness trends.
Required Skills
- Active Registered Nurse (RN) license in good standing (in any US state, with willingness to obtain additional state licenses as required).
- Minimum of 3 years of clinical nursing experience.
- Strong understanding of health behavior change theories and motivational interviewing techniques.
- Excellent communication, interpersonal, and active listening skills.
- Proficiency in using virtual communication platforms and electronic health records (EHR) systems.
- Ability to work independently and manage a caseload effectively in a remote environment.
- Demonstrated empathy, patience, and cultural sensitivity.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN) or higher.
- Certification in Health Coaching (e.g., NBHWC, ADAPT, Wellcoaches).
- Experience in telehealth, remote patient monitoring, or virtual health coaching.
- Knowledge of various chronic disease management protocols.
- Familiarity with population health management strategies.
